Job Summary:

The Medical Social Worker provides support and assistance to patients and their families in coping with medical, emotional, financial, and social challenges related to illness, hospitalization, and treatment. The role involves assessing patient needs, facilitating access to resources, and collaborating with healthcare teams to ensure holistic patient care.

Key Responsibilities:

  • Conduct psychosocial assessments of patients and families.
  • Provide counseling and emotional support to patients dealing with illness, trauma, or stress.
  • Assist patients and families in understanding and coping with diagnoses, treatment options, and care plans.
  • Coordinate with doctors, nurses, and other healthcare professionals to develop comprehensive patient care strategies.
  • Identify patients' financial, social, or emotional needs and connect them with appropriate resources (government programs, NGOs, community services, financial aid, etc.).
  • Facilitate discharge planning, including referrals for home care, rehabilitation, and community-based services.
  • Provide crisis intervention during emergencies or critical cases.
  • Advocate for patient rights and ensure ethical and compassionate care.
  • Document case records, assessments, and interventions in compliance with hospital policies.
  • Participate in health education programs, patient support groups, and hospital committees.

Qualifications:

  • Bachelor's degree in Social Work (BS Social Work).
  • Licensed Social Worker (Registered Social Worker) in the Philippines.
  • Preferably with experience in a hospital, clinical, or healthcare setting.
  • Strong communication, counseling, and problem-solving skills.
  • Ability to work with multidisciplinary teams.
  • Compassionate, patient-centered, and resourceful.

Job Types: Full-time, Permanent

Pay: Php17, Php18,000.00 per month
