Our organization seeks a highly skilled Intake Coordinator to oversee the intake process in our fast-paced home health environment. This critical role serves as a bridge between physicians, nurses, patients, and administrative staff, ensuring seamless operations.
Key Responsibilities- Coordinate new patient intake processes in accordance with CMS and California state regulations
- Collaborate with physicians and nurses to review, write, and edit clinical notes
- Manage referrals, authorization statuses, and admission workflows on a daily basis
- Support administrative operations, including scheduling, documentation, and internal communications
- Serve as the primary liaison for field clinicians and doctors to streamline documentation and provide timely patient care
- Utilize DataSoft Logic (DSL) and Axxess systems for documentation, compliance, and reporting workflows
- Communicate effectively with patients, families, clinicians, and administrative teams
- 3-5 years of experience in Intake Coordination in the United States, preferably within California's home health sector
- Clinical credentials: Registered Nurse (RN), Doctor (MD), or high-level clinical healthcare expert
- 1-2 years of administrative experience in healthcare—ideally in home health
- Flawless English communication skills—both written and spoken—with a clear and understandable accent
- Proven hands-on experience with DataSoft Logic and Axxess platforms
- Familiarity with Medicare/Medi-Cal intake protocols, Plan of Care documentation, and OASIS standards
