Job DescriptionDuties and Responsibilities:
- Assist patients in accessing care and navigating hospital services. Assess barriers to care and engage patients and families in creating potential solutions to hospital-related challenges.
- Guide the patient through the health care system and work to overcome obstacles to receiving care and treatment.
- Identify appropriate and credible resources responsive to patient needs (clinical or non-clinical).
- Educate patients and families on the multidisciplinary nature of care, roles of team members, and what to expect from the health care system. Refer to clinical staff to answer questions about clinical information, treatment choices and potential outcomes.
- Empower patients to communicate preferences and priorities for treatment to their health care team; facilitate shared decision making.
- Reduce barriers to timely treatment by identifying patient needs and directing them to sources of emotional, financial, administrative, or cultural support.

- Collaborate with different groups, health care teams, and resource providers.
- Build professional relationships with the health care team and learn the role of each team member to facilitate patient care.
- Maintain a high level of confidentiality for patient information.
- Follow up with patients to support adherence to the treatment plan through non-clinical barrier assessment and referrals, in collaboration with the clinical team. Contribute to patient navigation program development, implementation, and evaluation.
- Help patients optimize time with their doctors and treatment team (e.g., prioritize questions, clarify information).
- Encourage active communication between patients, families, and health care providers to optimize outcomes.
Minimum Qualifications:
- Graduate of a Bachelor's Degree in a customer service related field (e.g., BS Hotel, Restaurant and Management, etc.).
- Experience in customer service; excellent customer service skills and active listening.
- Computer literate in Windows-based applications; basic understanding of medical terminologies, hospital operations, and payment processing is an advantage.
- Willing to work in shifting schedule and onsite in Bonifacio Global City, Taguig.
