We are seeking an experienced and detail-oriented Hospice Team Leader (TL) to join our dynamic team. The TL plays a key role in ensuring the efficient, accurate, and timely completion of clinical documentation audits by guiding and supporting Clinical Documentation Associates (CDAs).

This role requires strong leadership, problem-solving, and communication skills, with a passion for quality and compliance in hospice documentation. The ideal candidate thrives in a fast-paced environment and enjoys mentoring others toward operational excellence.

RESPONSIBILITIES:

Daily Deliverables

• Chart Assignment & Oversight: Distribute balanced chart workloads and ensure CDAs understand chart type, priorities, and client-specific requirements
• Production Monitoring: Review and verify CDA production logs for accuracy; correct or escalate discrepancies promptly
• Real-Time Support: Serve as the first point of contact for CDA questions, EMR issues, or unclear documentation. Coordinate with QA or EMR teams to resolve blockers
• Spot Quality Checks: Perform daily quality reviews of sample charts to identify trends or training opportunities
• Attendance & Shift Monitoring: Validate CDA attendance, engagement, and timely logins throughout shifts
• Internal Communication: Cascade updates, SOP changes, or client instructions to team members and ensure full comprehension
• Task Support: Monitor completion of team side tasks, report participation, and EMR coordination activities

Weekly Deliverables

• Performance Pulse Checks: Review weekly productivity, accuracy, and compliance metrics for each CDA. Provide targeted coaching or recognition as needed
• Team Huddles: Lead or co-lead team meetings to discuss updates, share insights, and reinforce best practices
• Issue Escalation Summary: Track and summarize recurring documentation or process issues for management review
• Time-Motion Study Support: Facilitate CDA participation in time-motion sampling and ensure proper tracking
• Documentation Feedback: Gather CDA input on unclear or conflicting documentation and relay to QA/EMR teams

Monthly Deliverables

• Team Performance Summary: Prepare a concise monthly report highlighting trends, accomplishments, and challenges
• Evaluation Support: Contribute observations for CDA performance reviews, including feedback on quality, compliance, and professionalism
• Process Reinforcement: Conduct refresher sessions or review job aids to ensure continued SOP adherence
• Team Engagement: Recommend and support morale-boosting initiatives and monitor team sentiment for early signs of burnout
• Cross-Department Coordination: Serve as liaison between QA, EMR, and scheduling teams when Operations Management is unavailable

REQUIREMENTS:

• Active Registered Nurse (RN) license (required)
• Minimum 2 years of hospice experience, with proven accuracy in clinical documentation
• Strong expertise in hospice operations and proficiency with the HOPE tool
• Background in auditing or quality assurance (strongly preferred)
• Demonstrated leadership or supervisory experience
• In-depth knowledge of CMS hospice regulations and documentation standards
• Excellent analytical, organizational, and communication skills
• EMR proficiency required
• Willingness to work night shifts and can start immediately
