Rate: $5/hr and earn up to PHP 50,000 +/month
Schedule: Monday - Friday: 8:00 am - 8:00 pm PST, with 1 hour unpaid Lunch break

HOME HEALTH HOSPICE INTAKE/SCHEDULING COORDINATOR (WORK FROM HOME)
This position is somewhat similar to Home Health Intake/Scheduler, but has few more critical tasks. This person must be very nimble to be able to jump between tasks as Hospice level of care requires immediate actions in certain situations. This person will be under constant monitoring by the Director of Patient Care Services for Hospice and be this person's "right hand" for patient support items. This person must be mature, reliable and fully committed. It would be a huge plus if this person is RN, but not required. Huge plus if this person is able to work extra shifts on weekends and stay later if needed to finish critical tasks.
Qualifications:
- Must have Hospice experience.

Responsibilities:
- Answers calls from patients and clinicians related to Hospice care
- Enters incoming referrals patient' information from hospitals, facilities, and providers into Kinnser, CareStitch and other systems
- Communicate with patients and families about services, scheduling, and care start dates
- Assigns visits to clinical team and schedule visits
- Processes referrals from community liaisons to support timely admissions
- Communicates with patients and families about details of admission
- Schedules home visits for clinicians to see patients
- Responsible for informing staff of any changes, updates, etc., regarding admissions; discharges, visits, schedules; and clinical meetings as directed by the Clinical Director/Director of Patient Care Services and/or Administrator.
- Prepares, sends and collect electronic admission packets.
- Ensures that admission consents are returned to the office, uploaded to all electronic systems
- Ensures that all physicians orders, including CMS 485 (Plan of Care) are sent to the physicians for signature and returned to the office with physician signature.
- Monitors unsigned physicians' orders and submits additional request for signed orders to physicians offices.
- Ensures completeness of medical records of discharged patients.
- Places DME orders with clinicians, patients, physicians and DME suppliers.
- 1Assists in obtaining and documenting of prior authorization for services from insurance providers
- Performs other necessary functions/duties as assigned by the Clinical Director/Director of Patient Care Services and/or Administrator.
Job Type: Full-time
Pay: Php292.99 per hour
Expected hours: 40 per week
