We're seeking a Home Health Team Leader (TL) to provide hands-on leadership and operational support to our Clinical Documentation Auditors (CDAs). This role is central to maintaining accuracy, timeliness, and compliance across our clinical documentation review process. The ideal candidate has strong leadership skills, a background in home health or clinical documentation, and a passion for driving quality and productivity in a remote team environment.
Overall Objective
The Team Leader supports the efficient, accurate, and timely execution of clinical documentation audits by providing daily guidance, workload management, real-time troubleshooting, and performance coaching to assigned CDAs. You will serve as the frontline facilitator of quality and productivity, ensuring alignment with client protocols and operational goals.
RESPONSIBILITIES:
Daily Deliverables
- Chart Assignment & Oversight:
Ensure balanced chart distribution, confirm understanding of priorities or client-specific instructions, and flag any workload imbalances
- Production Monitoring:
Review real-time production logs, correct inaccuracies, and ensure charts are tagged and logged per SOP
- Real-Time Support:
Provide immediate assistance for documentation questions or EMR blockers; coordinate with QA or EMR teams as needed
- Spot Quality Checks:
Review sample charts daily to ensure accuracy and adherence to guidelines; escalate concerns to QA
- Attendance & Shift Monitoring:
Validate timely logins, active engagement, and report attendance variances
- Internal Communication:
Cascade critical updates and SOP changes, ensuring team acknowledgment and understanding
- Support Task Completion:
Monitor progress on side tasks and follow up on shared responsibilities (e.g., report drafts, EMR coordination)
Weekly Deliverables
- Conduct performance pulse checks for each CDA, providing feedback and coaching where needed
- Lead team huddles to address issues, share updates, and promote peer learning
- Prepare issue escalation summaries for recurring documentation or EMR challenges
- Collaborate in time-motion studies and ensure accurate tracking by team members
- Gather and submit feedback on documentation clarity to QA or EMR teams
Monthly Deliverables
- Prepare a team performance summary, highlighting trends, challenges, and recognition opportunities
- Support evaluation processes, contributing input for probationary or quarterly reviews
- Reinforce process compliance through mini-refreshers or SOP reviews
- Promote team engagement, recognizing achievements and addressing burnout risks early
- Act as point of contact for EMR, QA, and scheduling teams when Operations is unavailable
REQUIREMENTS:
- Active Registered Nurse (RN) license (required)
- Minimum 2 years of home health experience, with proven accuracy in clinical documentation
- 1–2 years of leadership experience in a Home Health setting
- Knowledge of ADR/PCR processes, survey preparation, and/or QAPI (strong advantage)
- Proficient in EMR systems, chart review, and documentation standards
- Skilled in MS Office and performance tracking tools
- Exceptional leadership, communication, and coaching abilities
- Strong coordination skills with cross-functional/operations teams
- Willingness to work night shifts and can start immediately
