Family Care Coordinator Role

We are seeking a highly motivated and empathetic individual to join our team as a Family Care Coordinator. In this role, you will play a vital part in empowering underserved families to access care by guiding them through the documentation process.

Job Description:

  • Collect, review, and track required documentation from caregivers and families (e.g., IDs, certifications, health records).
  • Make outbound calls and respond to inbound inquiries to support families throughout the documentation process.
  • Handle high-volume phone interactions daily while maintaining professionalism, empathy, and efficiency.
  • Ensure all documents are accurate, complete, and compliant with relevant regulations.
  • Communicate clearly and compassionately with families via phone, text, and email.
  • Maintain organized records and track expiring documents, sending timely reminders.

Requirements:

  • Extensive experience in customer service, administrative, or healthcare support roles.
  • Proven ability to handle high-volume inbound and outbound calls in a fast-paced environment.
  • Excellent organizational skills and strong attention to detail when reviewing documents.
  • Clear and professional communication skills, both verbal and written.
  • Familiarity with CRM systems, shared drives, or document tracking tools.
  • A compassionate and patient approach when working with families and caregivers.
