Family Care Coordinator Job Description

We are seeking a compassionate and organized Family Care Coordinator to join our team. As a key member of our family support services, you will play a vital role in empowering families to access quality care for their loved ones.

About the Role:

  • This is a full-time remote position with a regular working schedule of 9:00AM - 5:00PM EST.
  • You will work closely with our onboarding, credentialing, and family support teams to ensure a seamless process for families and caregivers.

Responsibilities:

  1. Collect, review, and accurately track required documentation from caregivers and families.
  2. Conduct outbound calls and respond to inbound inquiries, providing clear guidance to families throughout the documentation process.
  3. Manage a high volume of daily phone interactions (60–70 calls), demonstrating professionalism, empathy, and efficiency.
  4. Verify that all documentation is accurate, complete, and compliant with state regulations and Medicaid/Medicare requirements.
  5. Communicate effectively and compassionately with families via phone, text, and email to ensure a supportive experience.
  6. Maintain well-organized records, monitor upcoming expirations, and send timely reminders to prevent lapses.

Requirements:

  1. Extensive experience in customer service, administrative support, or healthcare-related roles.
  2. Proven ability to manage high-volume inbound and outbound calls in a fast-paced environment.
  3. Strong organizational skills with exceptional attention to detail in document review and tracking.
  4. Clear, professional communication skills, both verbal and written.
  5. Proficiency with CRM systems, shared drives, and/or document management tools.
