We are seeking an experienced and dedicated Clinical Coordinator (RN) to join our home health care team. This role is responsible for leading and supporting quality improvement efforts across the organization, ensuring clinical excellence, regulatory compliance, and positive patient outcomes. The Clinical Coordinator plays a key role in evaluating and improving care delivery, providing clinical oversight, and fostering a culture of continuous quality improvement.

Responsibilities:

Quality Improvement & Clinical Oversight

  • Implement and monitor the organization's Quality Improvement (QI) program.
  • Review daily clinician notes and Plans of Care (485s) for completeness and accuracy.
  • Monitor and analyze clinical documentation to evaluate Outcome-Based Quality Improvement (OBQI) measures.
  • Perform QAPI studies and data collection related to clinical record reviews.
  • Compile, trend, and report quality data related to patient care, safety, infection control, risk management, outcomes, and satisfaction.
  • Collaborate with the Quality Management Director to ensure documentation standards are met.

Safety, Risk Management, and Infection Control

  • Support the coordination and education of staff in safety and infection control procedures.
  • Review incident reports for accuracy and follow-up; maintain a trending log and recommend process improvements.
  • Stay current with home care safety and infection control standards.

Regulatory & Accreditation Compliance

  • Maintain up-to-date knowledge of Medicare Conditions of Participation and Joint Commission standards.
  • Ensure internal policies reflect current local, state, and federal regulations.
  • Recommend policy updates as needed to ensure compliance.

Clinical Support & Staff Development

  • Act as a clinical resource and mentor for field staff.
  • Educate staff on quality improvement initiatives and documentation best practices.
  • Provide training on the performance improvement plan and team development processes.

Requirements:

  • Active RN license
  • 2–3 years of clinical experience (home health preferred)
  • Strong knowledge of quality improvement, utilization management, and clinical documentation
  • Excellent communication, coordination, and organizational skills
  • Self-motivated; able to work independently and collaboratively
  • Must be amenable to start ASAP
