Position: Clinical Care Coordinator (Licensed Practical Nurse/Registered Nurse)

Job Summary:

Looking for a dedicated Clinical Care Coordinator to oversee and optimize patient care. This role involves coordinating patient care plans, and ensuring compliance with healthcare regulations. The ideal candidate will have an experience in clinical coordination or case management, and strong organizational and communication skills.

Key Responsibilities:

  • Coordinate and oversee patient care plans to ensure quality and efficiency.
  • Act as a liaison between patients, healthcare providers, and support staff.
  • Monitor patient progress, assess care needs, and implement necessary adjustments.
  • Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
  • Answer all incoming calls and direct them to the appropriate personnel when needed
  • Follow up on faxes/orders that have not been returned/signed within 7 days. Refax nor notify the office staff to follow up in person
  • Coordinate with Scheduling team as needed
  • Efax visit notes
  • PCP number/fax confirmation

Qualifications:

  • Previous experience in clinical coordination, case management, or a similar role.
  • Strong knowledge of healthcare regulations and patient care best practices.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to work effectively in a fast-paced environment and handle multiple priorities.
