Objective:

Ensure timely resolution of clinical documentation and QA-related open items to support compliance, operational efficiency, and downstream processes.

Proposal:

Hire 1 Full-Time Employee (FTE) for a Clinical Coordinator role dedicated to managing open clinical items.

Key Responsibilities:

  • Run a standardized weekly report to identify open items across clinicians.
  • Follow up with relevant stakeholders—Clinicians, DON, Clinical Manager, Ida Coding, Cliniqon—regarding:
  • OASIS, POC, Note completion
  • Signature status
  • Coding completion
  • QA completion
  • Action items completion
  • OASIS, POC, Note approval
  • Use multi-channel communication (calls, texts, emails) to ensure accountability and timely closure.
  • Escalate persistent delays or systemic issues to leadership for resolution.
  • Maintain a tracking dashboard to monitor progress and flag high-risk delays.

Impact:

  • Reduces bottlenecks in documentation and QA workflows.
  • Improves visibility and accountability across clinical teams.
  • Supports readiness for billing and compliance audits.

Clinical Coordination for Billing Priority

Objective:

Accelerate billing readiness by proactively resolving patient-specific open items that impact revenue cycle timelines.

Proposal:

Hire 1 Full-Time Employee (FTE) for a Billing-Focused Clinical Coordinator role.

Key Responsibilities:

  • Run a billing readiness report on a regular cadence (e.g., bi-weekly or aligned with billing cycles).
  • Review each patient listed to identify and prioritize open items that must be completed for billing:
  • Missing documentation
  • Unsigned OASIS, POC, notes
  • Incomplete coding
  • QA not done
  • Unresolved action items
  • Approval not done
  • Coordinate with clinicians, vendors, and internal teams to expedite resolution of billing-critical items.
  • Use multi-channel communication (calls, texts, emails) to ensure accountability and timely closure.
  • Escalate persistent delays or systemic issues to leadership for resolution.
  • Maintain a tracking dashboard to monitor progress and flag high-risk delays.
