As a Care/Provider Navigator, you'll play a vital role in expanding our presence in the healthcare industry. Your primary responsibility will be to guide and support providers, ensuring seamless onboarding, engagement, and collaboration for improved patient outcomes.

This role focuses on coordinating care services and building strong relationships with healthcare providers. You'll work closely with our team to develop targeted outreach strategies, identify key opportunities, and drive business growth.

  • Key Responsibilities:
    • Care Coordination (80%):
      • Develop and maintain relationships with healthcare providers to ensure seamless care delivery.
      • Collaborate with cross-functional teams to design and implement effective care coordination strategies.
      • Identify and address care gaps, improving patient outcomes and satisfaction.
    • Provider Partnership (20%):
      • Build and maintain strong relationships with healthcare providers to drive business growth.
      • Develop and execute targeted outreach strategies to identify new opportunities and expand existing partnerships.

Required Skills and Qualifications:

  • Must-Haves:
    • Proven experience in care coordination or a related field.
    • Excellent communication and interpersonal skills.
    • Ability to work effectively in a fast-paced environment.
  • Nice-to-Haves:
    • Background in healthcare sales or marketing.
    • Experience working with electronic medical records (EMRs) and other healthcare technology systems.
