Primary Function / General Purpose of Position

The Team Lead, Transition and Business Transformation is responsible for leading and overseeing the execution of transition and transformation initiatives across business functions. This role manages a team of transition experts and coordinates with cross-functional stakeholders to ensure seamless knowledge transfer, process stabilization, and operational readiness. The position drives adherence to the Target Operating Model (TOM), ensuring full traceability from due diligence through hypercare, while delivering measurable business value, risk mitigation, and service continuity.

Essential Job Functions

  • Lead and manage a team of transition and transformation specialists to deliver projects within scope, budget, and timelines using PMBOK and/or Agile methodologies.
  • Oversee end-to-end transition activities, from due diligence and knowledge transfer to go-live and stabilization.
  • Drive process discovery and documentation efforts, ensuring creation and validation of process maps, SOPs, desktop procedures, and control points with Subject Matter Experts (SMEs) and Global Process Owners (GPOs).
  • Develop and monitor Knowledge Transfer (KT) strategies (e.g., train-the-trainer, shadowing, certification) and ensure proper documentation and validation through the Learning Management System (LMS).
  • Maintain and review RAID logs; escalate risks and issues with impact assessments and actionable mitigation plans; ensure timely resolution aligned with Service Level Agreements (SLAs).
  • Track and report transition metrics including business case baselining, productivity, quality, and cycle-time improvements.
  • Ensure project readiness through control testing, handoff validation, runbooks, and stabilization plans before go-live and during hypercare.
  • Prepare and present management reports, dashboards, and executive summaries, ensuring data accuracy and business insight.
  • Ensure team compliance with healthcare privacy (HIPAA) and data governance policies in all transition-related activities.
  • Lead change management and stakeholder communication initiatives; collect feedback and embed lessons learned into continuous improvement plans.
  • Coach, mentor, and develop team members, promoting a culture of accountability, collaboration, and process excellence.
  • Perform other job-related duties as required in support of organizational and departmental goals.

Licenses / Certifications

  • Lean Six Sigma Green Belt – Preferred
  • Project Management Professional (PMP) – Preferred

Education

  • Bachelor's degree in Business, Industrial Engineering, Management Engineering, Information Systems, or a related field – Required

Work Experience

  • Minimum 4 years of experience in transition and transformation, process improvement, or project management within a shared services or global business services (GBS) environment – Required
  • At least 1–2 years of experience in a lead or supervisory role – Preferred

Job Types: Full-time, Permanent

Pay: Php100, Php120,000.00 per month
