Job Title:

Remote USRN (California Licensed) – Full-Time | Philippines-Based

Position Summary:

The Remote USRN (California Licensed) will perform telehealth-based nursing functions, care coordination, documentation review, and support for California patients and clinical programs. The role requires adherence to California clinical and regulatory standards while collaborating virtually with an interdisciplinary U.S.-based care team.

This position is full-time and remote, operating on Pacific Standard Time (PST) from 8:00 AM to 6:00 PM, Monday through Friday.

Key Responsibilities:

  • Conduct telephonic and virtual patient assessments, care plan reviews, and follow-ups for California members.
  • Support CalAIM, Enhanced Care Management (ECM), and Community Supports programs by coordinating care transitions, documenting interventions, and ensuring continuity of services.
  • Collaborate with U.S.-based nurses, social workers, and care managers to ensure compliance with California BRN, HIPAA, and DHCS requirements.
  • Review medical records, H&P/physician notes, and nursing facility documentation for completeness and accuracy.
  • Participate in interdisciplinary team meetings via Zoom or Teams.
  • Monitor patient progress, update documentation within EHR systems, and escalate high-risk cases appropriately.
  • Provide patient and caregiver education on health conditions, medications, and available support resources.
  • Maintain daily productivity, clinical accuracy, and confidentiality standards.

Qualifications:

  • Active U.S. RN license with California Board of Registered Nursing (CA BRN) or CA endorsement approval (required).
  • Currently residing in the Philippines.
  • Minimum 2 years of clinical experience (hospital, home health, assisted living, or care management preferred).
  • Experience with telehealth, case management, or care coordination strongly preferred.
  • Excellent English communication skills (verbal and written).
  • Reliable internet connection, laptop/desktop, webcam, and quiet workspace.
  • Familiarity with U.S. healthcare documentation, HIPAA compliance, and EHR systems.

Work Schedule:

  • Full-time, remote position
  • 8:00 AM – 6:00 PM Pacific Standard Time (PST)
  • Monday – Friday
