Job Purpose (Summary)

Provide patient-centered health coaching for RPM and CCM active patients to improve self-management, adherence, and outcomes. Conduct scheduled and ad hoc coaching calls, interpret trends in vitals and symptoms, reinforce care plans, and coordinate escalations to clinical staff when risk thresholds are met. Ensure accurate documentation in EHR and Pipeline and deliver a supportive experience that drives sustained compliance and reduces avoidable utilization.

Key Responsibilities (Main Duties)

  • Conduct outbound and inbound coaching sessions that build patient motivation, set SMART goals, and reinforce care plans for chronic conditions.
  • Review vitals trends, symptoms, and red flags, and provide evidence-based self-management education tailored to literacy and culture.
  • Achieve required interactive minutes each month for RPM and CCM and schedule the next touchpoint.
  • Close the loop on risk alerts by following escalation protocols to nurses or providers and documenting actions and outcomes.
  • Coordinate with Logistics and Clinical on replacements, medication updates, and care-plan changes; track tasks to closure.
  • Document every interaction in Pipeline with accurate codes, minutes, goals, barriers, and follow-up plans.
  • Contribute insights to Operations, QA, and Training about recurring barriers, social needs, and content improvements.
  • Protect PHI, follow HIPAA and TCPA rules, and adhere to schedule, QA standards, and scripting where applicable.

Qualifications

  • B2 English Profile or Higher
  • Bachelor's Degree in Medicine, Nutrition, Public Health, Nursing, Psychology, or related field, or equivalent clinical experience.
  • Equivalent certificate in the US. (USMLE, NAPLEX, NCLEX, ETC…)
  • Minimum 1 to 2 years in working in the medical field.
  • Experience with RPM or CCM programs in a US payer context and familiarity with hypertension, diabetes and obesity management. (Preferred but not required)
  • Can work fixed US shifts 9AM - 6PM EST.

Technical Skills

  • Competent interpreting RPM data for BP, glucose, weight, and oximeter with understanding of normal ranges, trends, and artifact.
  • Comfortable guiding basic device use and app workflows.
  • Working knowledge of social determinants screening, care-gap reminders, and medication reconciliation basics.
  • Strong Excel or Google Sheets familiarity for trackers.

Soft Skills / Behavioral Competencies

  • Empathy, clear communication, and teach-back that match senior and multi-condition populations.
  • Motivational Interviewing, goal setting, and barrier removal with high follow-through.
  • Structured problem solving, sound judgment, and calm de-escalation.
  • Cultural sensitivity, privacy mindset, and teamwork with Clinical, Tech Support and Operations.
