Job Description:

  • Conduct interview to all seeking admission/treatment to determine eligibility for MSS assistance.

  • Conduct regular ward visits/rounds to the patient.

  • Conduct collateral interview through home visitation and follow-up to abandoned and referred cases.

  • Refer and conducts abandoned patients, pickup, homeless and mentally-ill patients to other agencies for institutionalization.

  • Attend to patients medical needs as well as complaints.

  • Provide outright counselling with patients and relatives before discharge of patients in the hospital.

  • Prepare referrals/letters and other communication necessary for networking, linkages and coordination with community resources.

  • Record progress notes/reports regularly for abandoned and referred patients.

  • Perform other related functions as assigned by the immediate supervisor.

Minimum Qualifications: Qualifications:

  • Must be a Registered Social Worker
  • With valid license
