Chronic Care Manager (CCM)

We're seeking a compassionate and detail-oriented Chronic Care Manager who is passionate about empowering patients with chronic conditions to live healthier lives.

This fully remote role involves connecting with patients, guiding them in managing their conditions, and making a real impact on their health outcomes.

Key Responsibilities
  • Make monthly outreach calls to patients enrolled in our CCM program.
  • Collaborate with patients and providers to design and update individualized care strategies.
  • Monitor symptoms, identify risks, and escalate care before complications arise.
  • Coordinate care by working seamlessly with primary care providers, specialists, and community resources.
  • Patient Education: Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.
  • Document patient records accurately and timely in our EHR system.
Requirements
  • Valid RN license.
  • Minimum 1 year of clinical nursing experience, preferably in chronic care, case management, or primary care.
  • Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure.
  • Excellent communication, critical thinking, and organizational skills.
  • Comfortable with EHR systems and virtual care tools.
Bonus Points If You Have
  • Experience in motivational interviewing or other patient engagement methods.
