Job Summary

We are seeking a skilled and compassionate Clinical Navigator to join our team. The successful candidate will play a vital role in delivering high-quality care to clients, ensuring seamless navigation through the treatment process.

Duties and Responsibilities
  • Act as the primary point of contact for assigned outpatient clients and their families, providing support with non-clinical needs and enhancing treatment outcomes.
  • Coordinate all therapy and psychiatric appointments, communicating proactively with providers to determine service frequency and confirming client attendance.
  • Document all appointments and manage calendars accurately across clinical schedules.
  • Track and report no-shows, cancellations, and reschedules, notifying finance of applicable charges for billing.
  • Maintain ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.
  • Identify barriers and needs related to housing, education, transportation, healthcare, legal services, and other social determinants of health, linking clients to appropriate resources.
  • Participate in interdisciplinary team meetings to review and support treatment plan goals.
  • Escalate urgent or critical issues to the Clinical Director immediately.
  • Uphold professional ethics and confidentiality standards in all communications and records.
Requirements
  • 2+ years of experience in behavioral health, social services, or case management.
  • Strong written and verbal communication skills.
  • Proficiency in Google Workspace and/or Microsoft Office.
  • Reliable high-speed internet and ability to work from a secure remote location.
  • Ability to obtain a Level One Fingerprint Clearance Card.
Preferred Qualifications
  • Associate's or Bachelor's degree in Social Work, Psychology, or related field.
  • Experience working with adolescents and families.
  • Knowledge of Medicaid, state behavioral health systems, and community-based services.
