Job Overview

We are seeking a skilled Care Coordinator to play a vital role in enhancing patient health outcomes.

In this critical position, you will serve as the primary care coordinator for assigned patients, conducting outreach and assessments to develop personalized care plans. Your responsibilities will include educating patients on disease management, tracking key health indicators, and intervening early to prevent unnecessary hospital visits.

  • Serve as the primary care coordinator for assigned patients
  • Conduct telephonic outreach and/or in-person assessments (based on program guidelines)
  • Collaborate with physicians, specialists, and team members to create individualized care plans
  • Educate patients on disease management and track key health indicators
  • Intervene early to prevent ER visits or hospital readmissions
Requirements and Qualifications

To be successful in this role, you should possess:

  • An active RN license
  • Experience in chronic care, patient education, or case management
  • Excellent English communication skills
  • Strong clinical judgment and attention to detail
