Your Role in Patient-Centered Care
Be part of our client's team as a Clinical and Operations Support Specialist, where you'll handle patient communications, support device and platform use, maintain clinical records, coordinate referrals, and prepare reports-all while ensuring patients receive timely, personalized, and seamless care.

What You'll Need to Succeed
- 3 to 5 years of relevant experience
- Prior background in healthcare, call center, or patient coordination preferred
- Strong organizational and multitasking skills
- Excellent communication and customer service abilities
- Comfort with technology platforms such as EHRs, CRM tools, and messaging systems
- Detail-oriented, proactive, and able to thrive in a fast-paced startup environment
What You'll Do Every Day
Patient Communications & Call Management
- Answer, document, and relay incoming calls; transfer messages as appropriate
- Send SMS reminders and follow-ups to improve patient engagement and adherence
- Support re-engagement workflows for patients with low activity and manage inactivation or offboarding processes
- Provide non-clinical support such as portal login troubleshooting and basic care guidance
Device & Platform Support
- Assist with initial device setup, troubleshooting, and return/recycling processes
- Track device inventory and coordinate distribution and return kits
Clinical Data & Chart Maintenance
- Update ISHI and clinic records (medications, labs, EF, clinical events) regularly
Care Coordination & Referral Support
- Manage referrals by onboarding new patients into ISHI systems
- Support scheduling of labs, procedures, and follow-up appointments
- Facilitate secure exchange of clinical records between ISHI and partner clinics
Reporting
- Upload monthly reports
