Job Description:

  • Review and process physician and NP orders in multiple EHR systems.
  • Enter and track orders across skilled nursing facilities (SNFs) and home health agencies (HHAs).
  • Verify order execution and follow up on completion.
  • Act as primary nursing contact for order-related communications.
  • Clarify orders with physicians as needed.
  • Coordinate with Telehealth Care Coordinators to align orders with patient care plans.
  • Maintain accurate documentation and ensure compliance with HIPAA/CMS standards.
  • Support audits and quality reviews with order-related data.
  • Follow team and/or clients proper procedures, policies, and methodologies as instructed.
  • Performs other related duties as necessary or assigned

Minimum Qualifications:

  • Active RN or USRN license.
  • Clinical experience in acute, post-acute, or home health settings preferred.
  • Strong attention to detail and organizational skills.
  • Excellent communication and coordination abilities.
  • Proficient with electronic health records (EHR) and documentation.
  • Ability to manage multiple tasks in a fast-paced, remote telehealth environment.
  • Interest in medical informatics, data analysis, or quality improvement is a plus.
  • Able to work from home with reliable internet and a backup connection.
