Virtual Care Coordinator (Remote | Full-Time)

About the Role

We are seeking a compassionate and detail-oriented Virtual Care Coordinator to join our healthcare team. In this role, you'll serve as the main point of contact for patients managing Diabetes and Fatty Liver Disease, ensuring they receive coordinated, timely, and supportive care. You'll work closely with physicians, nurse practitioners, and dietitians to help patients achieve better health outcomes through proactive communication and follow-up.

Key Responsibilities

  • Coordinate treatment plans, lab tests, follow-ups, and referrals.
  • Educate patients on disease management, lifestyle modifications, and medication adherence.
  • Conduct virtual check-ins to track progress and address barriers.
  • Assist with appointment scheduling, reminders, and navigation of healthcare systems.
  • Monitor and document patient-reported outcomes (blood sugar logs, weight tracking, dietary habits, etc.).
  • Identify and escalate high-risk cases to providers.
  • Support patient enrollment in chronic care management (CCM) and remote patient monitoring (RPM) programs.
  • Maintain accurate, HIPAA-compliant records in EMR systems.

Qualifications

  • Experience in patient care coordination, nursing, case management, or related healthcare fields.
  • Strong knowledge of Diabetes management and awareness of Fatty Liver Disease (NAFLD/NASH).
  • Excellent communication skills with the ability to explain medical concepts clearly.
  • Proficiency with telehealth platforms and EMR systems.
  • Strong organizational and multitasking abilities.

Preferred

  • Medical Assistant, LPN, RN, or CDCES certification.
  • Prior experience with CCM or RPM programs.
  • Bilingual skills are a plus.
