Role Description
- Conduct outbound and inbound coaching sessions that build patient motivation, set SMART goals, and reinforce care plans for chronic conditions.
- Review vitals trends, symptoms, and red flags, and provide evidence-based self-management education tailored to literacy and culture.
- Achieve required interactive minutes each month for RPM and CCM and schedule the next touchpoint.
- Close the loop on risk alerts by following escalation protocols to nurses or providers and documenting actions and outcomes.
- Coordinate with Logistics and Clinical on replacements, medication updates, and care-plan changes; track tasks to closure.
- Document every interaction in Pipeline with accurate codes, minutes, goals, barriers, and follow-up plans.
- Contribute insights to Operations, QA, and Training about recurring barriers, social needs, and content improvements.
- Protect PHI, follow HIPAA and TCPA rules, and adhere to schedule, QA standards, and scripting where applicable.
Qualifications
- Proficiency in English (B2 or above)
- Bachelor's Degree in Medicine, Nutrition, Public Health, Nursing, Psychology, or related field, or equivalent clinical experience.
