Your Role
As an Care Coordinator AU, you are responsible for:
Role Purpose The Care Coordinator supports older adults receiving Home Care Package and Support at Home services by managing onboarding, service coordination, scheduling, referrals, documentation, and client communication. The role ensures services are safe, goal-aligned, culturally inclusive, and responsive to each client's needs. It also monitors wellbeing, identifies early deterioration, assists with risk assessments, and supports care plan reviews in line with wellness, reablement, and dignity-of-risk principles.
Key Responsibilities
1. Client Onboarding
- Complete client intake and system setup
- Collect health, cultural, communication, and support information
- Explain program details, service options, rights, responsibilities, and pricing
- Apply trauma-informed, culturally safe, and supported decision-making principles
- Identify early risks and escalate appropriately
- Assist clients with onboarding and care planning documents
2. Care Planning & Reassessment
- Gather information across functional, clinical, social, cultural, and safety areas
- Support development and review of Support Plans and quarterly budgets
- Coordinate reassessments when needs or risks change
- Assist in documenting risk themes and mitigation strategies
- Ensure updates reflect wellness and reablement principles
3. Monitoring Wellbeing & Risks
- Conduct early-stage monthly touchpoints and additional monitoring as required
- Track wellbeing through communication with clients, families, and providers
- Identify early deterioration and escalate promptly
- Support updates to care plans, risk profiles, and safety plans
- Record incidents and assist with documentation
4. Service Coordination, Scheduling & Referrals
- Organise timely commencement of services
- Communicate preferences and needs to rostering teams
- Liaise with support workers, clinicians, and external providers
- Issue referrals, coordinate aids/equipment, and support continuity of care
- Facilitate transitions to/from hospital, respite, or short-term programs
5. Client & Family Communication
- Provide clear, trauma-informed and culturally respectful communication
- Support decision-making and offer interpreters or aids where needed
- Assist with feedback processes
- Maintain accurate, real-time progress notes
6. Touchpoints & Feedback
- Conduct or coordinate monthly touchpoints
- Record feedback, concerns, and service themes
- Escalate issues and identify opportunities for goal-aligned service use
7. Budget & Financial Support
- Monitor budgets and flag risks of overspending
- Assist with consumables purchasing aligned with program rules
- Resolve billing queries with finance
- Prepare quarterly budget summaries
8. Quality, Safety & Compliance
- Maintain accurate documentation in Visual Care
- Support WHS, infection control, hazard reporting, and safe work practices
- Participate in audits, reviews, incident follow-ups, and improvement activities
- Ensure compliance with aged care legislation, quality standards, and organisational policies
9. Stakeholder Engagement
- Build positive relationships with clients, families, providers, and advocates
- Participate in case discussions, multidisciplinary meetings, and handovers.
Key Selection Criteria:
Communication
- Strong written and verbal English skills for client, team, and provider communication
- Healthcare or nursing background highly regarded
- Understanding of community or aged care services
Time Management
- Ability to manage multiple tasks, deadlines, and competing priorities
Interpersonal Skills
- Able to maintain positive, professional relationships with clients and stakeholders
Attention to Detail
- Accurate documentation of client information and service updates
Computer Proficiency
- Competent with general applications and willing to learn Visual Care software
