Job Title

Overview:

  • This role involves supporting the management of clinical operations in home care services.

The Remote Clinical Assistant will work closely with the Private Duty Nursing (PDN) Director of Nursing to ensure seamless clinical and administrative support.

Key Responsibilities:
  • Review patient care plans, MARs, and physician orders for accuracy and compliance.
  • Follow up with physicians for signed orders and ensure proper transcription into patient records.
  • Support scheduling workflows and help manage nursing visits to align with care plans.
  • Assist with client admissions and discharges by preparing required documentation.
  • Generate compliance reports and assist with audit readiness.
Care Coordination:
  • Monitor scheduling workflows to ensure PDN visits align with care plans.
  • Serve as a remote liaison between nurses, patients, families, and providers to streamline communication.
  • Track staffing assignments and availability to support resource allocation.
  • Ensure staff credentialing (licenses, CPR, TB, etc.) are tracked and up-to-date.
Quality Assurance & Compliance:
  • Support quality improvement initiatives by compiling reports and identifying documentation gaps.
  • Assist in reviewing clinical documentation for accuracy, completeness, and compliance with state/federal regulations.
  • Provide feedback on documentation corrections or updates to field nurses.
Training & Education Support:
  • Help facilitate remote orientation and training sessions for nursing staff.
  • Track completion of required in-services and continuing education.
  • Assist in distributing policy updates, clinical reminders, and compliance alerts to nursing staff.

Requirements:

  • Current LPN license or higher (RN preferred).
  • Associate's or Bachelor's degree in Nursing or related healthcare field strongly preferred.
  • 2+ years of clinical experience in home health, PDN, or related nursing environment.
  • Prior experience in administrative or clinical support roles preferred.
