
Supportive Client Advocate

At Remote Raven, we're seeking an experienced Client Advocate to provide exceptional support and guidance to individuals and families navigating the complexities of healthcare.
The ideal candidate will possess a deep understanding of the social determinants of health and be able to effectively connect clients with necessary resources. A strong communicator with excellent interpersonal skills, this individual will build trust and rapport with clients, empowering them to make informed decisions about their care.
Key Responsibilities:- Client Coordination: Serve as the primary point of contact for assigned clients, providing tailored support and facilitating communication between healthcare providers and family members.
- Schedule Management: Coordinate appointments, services, and referrals, ensuring seamless transitions and minimizing disruptions to client care.
- Documentation and Reporting: Maintain accurate and comprehensive records, tracking client progress and outcomes in accordance with established protocols.
- Collaboration and Communication: Foster effective partnerships with clinical staff, social workers, and community agencies to ensure holistic care and optimize client outcomes.
Requirements:
- A minimum of 2 years of experience in behavioral health, social services, or case management.
- Strong Written and Verbal Communication Skills: Ability to communicate complex information clearly and compassionately, tailoring messages to diverse audiences.
- Proficiency in Google Workspace and/or Microsoft Office: Experience with electronic systems and platforms to track and report client activities.
- Reliable High-Speed Internet: Access to secure remote work environments and ability to maintain confidentiality.
Preferred Qualifications:
- Associate's or Bachelor's Degree in Social Work, Psychology, or related field: Strong educational foundation in human behavior, social services, or psychology.
- Experience Working with Adolescents and Families: Understanding of the unique needs and challenges faced by these populations.
- Knowledge of Medicaid, State Behavioral Health Systems, and Community-Based Services: Familiarity with relevant laws, regulations, and service delivery models.
