The Role:Care Coordination (80%)
  • As a Care Coordinator, you will serve as the primary point of contact for patients, ensuring a professional and welcoming experience.
  • You will collect, verify, and update patient information accurately while maintaining confidentiality.
  • Schedule appointments efficiently and conduct initial screenings to direct patients appropriately.
  • Manage patient records in electronic medical record systems, adhering to HIPAA regulations.
  • Respond to patient inquiries via phone, email, or chat with empathy and clarity.
  • Provide pre-appointment instructions and ensure patients understand documentation and insurance requirements.
  • Coordinate with healthcare providers, insurance teams, and staff to facilitate seamless patient flow.
Provider Partnership (20%)
  • In this role, you will establish provider partnerships through outreach and present our company's value proposition.
  • High-volume outbound calling is required.
  • Maintain accurate outreach records in CRM software.
  • Track and report key performance indicators, such as calls, emails, meetings, and conversion rates.
  • Share insights on prospect feedback and market trends to enhance strategies.
Requirements:
  • Proven experience in care coordination, especially within physical healthcare settings.
  • At least one year of outbound sales experience, preferably in the healthcare industry.
  • Strong cold-calling and email outreach skills.
  • Proficiency in CRM tools, including HubSpot and LinkedIn Sales Navigator.
  • Excellent verbal and written communication skills.
  • Able to thrive in fast-paced, metrics-driven environments.
