I. Job Summary

A Patient Care Navigator serves as the primary liaison between patients, their families, and healthcare providers. This role is crucial in ensuring a positive patient experience by addressing concerns, helping, and facilitating clear communication. By collaborating with various departments, they work to continuously improve service quality and patient satisfaction

II. Duties and Responsibilities

  1. Serves as the primary contact for patients, addressing their needs, concerns, and inquiries.

  2. Facilitates clear communication between patients, their families, and healthcare providers.

  3. Provides accurate information to patients and their families about hospital services, policies, and procedures.

  4. Mediates between patients and healthcare staff to ensure a positive experience.

  5. Represents and advocates for patients' needs and concerns within the healthcare facility.

  6. Compliance with the quality procedures and policies.

  7. Other relevant tasks that may be required by the immediate supervisor.

III. Job Specifications:

1. Education/Experience:

Academic: Bachelor's degree in social work, nursing, or a related field

Experience: At least 2 years of experience in a healthcare or social services setting.

Licensure: N/A

Certification: N/A

2. Skills and Abilities:

a. Strong interpersonal and communication skills, understanding of medical terminology.

b. Ability to work with diverse populations.

c. Strong organizational skills to manage patient records and schedules.

d. Commitment to advocating patients' needs and removing barriers to care.
