Job Description

We are seeking a highly skilled and dedicated Senior Case Manager to join our team. This role involves providing case management for adults subject to community orders, focusing on safety, risk assessment, and rehabilitation.

The successful candidate will be responsible for operating in accordance with assessed risk and need, maintaining high standards of assessment to support effective supervision. They will also facilitate reintegration and improve rehabilitation outcomes for individuals under community-based orders.

Key responsibilities include:

  • Providing case management services to adults subject to community orders.
  • Conducting risk assessments and developing individualised plans to meet the needs of clients.
  • Collaborating with other professionals and stakeholders to ensure effective communication and coordination.
  • Facilitating reintegration and improving rehabilitation outcomes for individuals under community-based orders.
Required Skills and Qualifications

To be successful in this role, you will possess:

  • A relevant qualification in social work, psychology or a related field.
  • Experience in case management and/or community corrections.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • High level of organisation and time management skills.
