Job Description

The Outpatient Case Manager role entails providing remote, client-centered support throughout the treatment process. This involves ensuring efficient delivery and documentation of wraparound services by coordinating with clients, families, internal teams, and external service providers.

Key Responsibilities:
  • Act as the primary point of contact for assigned outpatient clients and their families.
  • Coordinate therapy and psychiatric appointments, including proactive communication with providers and clients.
  • Document appointments and ensure accurate calendar management across clinical schedules.
  • Track and report no-shows, cancellations, and reschedules.
  • Monitor client participation in scheduled services and follow up on attendance.
Qualifications:
  • Minimum 2 years of experience in behavioral health, social services, or case management.
  • Strong written and verbal communication skills.
  • Proficiency in Google Workspace and/or Microsoft Office.
  • Reliable high-speed internet access and ability to work from a secure remote location.
