
Outpatient Client Advocate

The Outpatient Case Manager plays a pivotal role in providing exceptional, client-centered support throughout the treatment process.
Key responsibilities include serving as the primary point of contact for assigned clients and their families, coordinating appointments, documenting appointments, tracking no-shows and cancellations, monitoring client participation in scheduled services, and maintaining ongoing communication and documentation regarding care coordination efforts.
Additionally, the case manager identifies barriers and needs across various social determinants of health, escalates urgent issues to the Clinical Director, and upholds professional ethics and confidentiality standards.
Requirements- 2+ years of experience in behavioral health, social services, or case management.
- Strong written and verbal communication skills.
- Proficiency in Google Workspace and/or Microsoft Office.
- Reliable high-speed internet and ability to work from a secure remote location.
- Ability to obtain a Level One Fingerprint Clearance Card.
