The Client Care Coordinator role is a critical position in our organization's mission to provide comprehensive support services to clients. We are seeking an experienced professional to join our team as a Client Care Coordinator.

Job Description

In this role, you will be responsible for providing client-centered support across all stages of treatment, including intake, active care coordination, appointment scheduling, and discharge planning. You will ensure the efficient delivery and documentation of wraparound services by coordinating with clients, families, internal teams, and external service providers. This includes tracking client progress, resolving barriers to care, and supporting treatment goals in accordance with the individualized treatment plan.

Some key responsibilities of the Client Care Coordinator include:

  • Serving as the primary point of contact for assigned outpatient clients and their families to support non-clinical needs and enhance treatment outcomes.
  • Coordinating all in-person and virtual therapy and psychiatric appointments, including proactive communication with providers to determine frequency of services and with clients to confirm attendance.
  • Documenting all appointments and ensuring accurate calendar management across clinical schedules.
  • Tracking and reporting no-shows, cancelations, and reschedules; notifying Finance of applicable no-show charges for billing.
  • Maintaining ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.
Required Skills and Qualifications

To be successful in this role, you will need to have the following skills and qualifications:

  • 2+ years of experience in behavioral health, social services, or case management
  • Strong written and verbal communication skills
  • Proficiency in Google Workspace and/or Microsoft Office
  • Reliable high-speed internet and ability to work from a secure remote location
