Scheduling and Care Coordination Specialist

As a highly organized and detail-oriented Scheduling and Care Coordination Specialist, you will play a critical role in managing clinician and client schedules, facilitating seamless care coordination, and overseeing key administrative and front-end billing tasks.

The ideal candidate is proactive, professional, and comfortable handling multiple responsibilities in a fast-paced environment, especially in a healthcare setting.

  • Scheduling Management
    • Develop and maintain accurate schedules for clinicians and clients to ensure optimal resource allocation and minimize downtime.
    • Ensure appropriate client-to-clinician ratios to provide quality care and service.
    • Serve as the primary point of contact for scheduling inquiries from clients and staff to resolve issues promptly and efficiently.
    • Assessments Coordination
      • Schedule assessments and reassessments in a timely and organized manner to meet regulatory requirements and maintain continuity of care.
      • Care Coordination
        • Coordinate care across multiple disciplines to provide comprehensive services and support to clients.
        • Arrange substitute clinicians when needed to ensure uninterrupted services and minimize disruption to clients.
        • Client & Staff Communication
          • Answer incoming calls and respond to inquiries with professionalism and urgency to provide exceptional customer service.
          • Maintain consistent and clear communication via phone and email with families and staff to build trust and rapport.
          • Record & Document Management
            • Maintain and audit client records for accuracy and compliance to ensure regulatory adherence and data integrity.
            • Ensure timely documentation and updates in relevant systems to maintain up-to-date information and facilitate smooth transitions.
            • Reception Oversight
              • Oversee receptionist functions, including managing calls, scheduling appointments, and addressing inquiries to provide seamless customer service.
              • Front-End Billing & Authorization Management
                • Verify insurance benefits and monitor ongoing eligibility to ensure accurate billing and reimbursement.
                • Track changes in coverage and authorization status to maintain up-to-date information and prevent delays.
                • Manage patient record reviews, collect client payments, and maintain up-to-date authorization tracking to streamline operations and improve cash flow.
                Requirements
                • Proven experience in scheduling, administrative support, or healthcare coordination.
                • Excellent communication and interpersonal skills to build strong relationships with clients, staff, and stakeholders.
                • Strong organizational and multitasking abilities to prioritize tasks, manage competing demands, and maintain attention to detail.
                • Proficiency in relevant software tools (Google Drive, CentralReach, Monday.com, Excel) to streamline operations and enhance productivity.
                • Prior experience in a healthcare or therapy-based setting is a plus.
