Care Coordination Opportunity

As a key member of our team, you will play a vital role in improving the health outcomes of patients across the US.

This is a full-time night shift role that requires a highly organized and compassionate individual who can work independently and as part of a remote team. You will be responsible for serving as the primary care coordinator for assigned patients, conducting telephonic outreach, and collaborating with healthcare professionals to create personalized care plans.

We are seeking an experienced Registered Nurse (RN) who possesses excellent English communication skills, strong clinical judgment, and attention to detail. If you have experience in chronic care, patient education, or case management, we would love to hear from you.

Key Responsibilities:

  • Serve as the primary care coordinator for assigned patients
  • Conduct telephonic outreach and/or in-person assessments
  • Collaborate with physicians, specialists, and team members to create individualized care plans
  • Educate patients and caregivers on disease management, medications, and lifestyle adherence
  • Track key health indicators and intervene early to prevent ER visits or hospital readmissions
  • Coordinate services across multiple healthcare providers, facilities, and support systems
  • Maintain timely, accurate records in electronic health records (EHRs)

Required Qualifications:

  • Active RN license
  • Experience in chronic care, patient education, or case management
  • Excellent English communication skills
  • Strong clinical judgment and attention to detail
