Job Description
  • Monitor remote patients and their data through various systems and use existing policies and procedures to address issues as they arise.
  • Ensure timely and accurate documentation of all relevant data across different systems.
  • Use protocols and system data to refer mothers to the correct resource or care partner.
  • Interact with the patients over the app, so that documentation is logged; detail any activities that patients do that aren't captured by the messages they send.
  • Assist other departments like Customer Service in helping resolve patient inquiries.
  • Identify clinical issues to be escalated by clinical supervisors in the US.
  • Deliver exceptional customer service and patient advocacy, with professionalism, confidentiality, and a focus on the experience and care of clients, partners, and stakeholders.
  • Able to handle emergent situations like high blood pressure or vaginal bleeding.
  • May send a once-a-month depression survey.
  • Check in on patients every week to ensure they are taking their medications or monitoring vital signs.
  • Actively engage in professional development to continuously improve existing skill sets; assist in updating and/or translating company documents like training, policy, reporting, and procedure.
  • Able to meet metrics/KPIs set by the client.

Responsibilities
  • Assist patients in accessing care and navigating hospital services; assess barriers to care and engage patients and families in creating solutions.
  • Guide patients through the health care system and overcome obstacles to timely treatment.
  • Identify appropriate resources responsive to patient needs (clinical or non-clinical).
  • Educate patients and families on the multidisciplinary nature of care; refer questions to clinical staff for information on treatment options and outcomes.
  • Empower patients to communicate treatment preferences and participate in shared decision making.
  • Reduce barriers to timely treatment by directing patients to emotional, financial, administrative, or cultural support.
  • Maintain confidentiality of patient information and follow up to support adherence to treatment plans in collaboration with the clinical team.
  • Contribute to patient navigation program development, implementation, and evaluation.
  • Help patients optimize time with doctors and treatment teams by prioritizing questions and clarifying information.
  • Encourage active communication between patients/families and health care providers to optimize outcomes.
Minimum Qualifications
  • Graduate of a Bachelor's degree in a customer service related field (e.g., BS Hotel, Restaurant and Management) or Nursing/medical allied fields as indicated per role.
  • Experience in customer service with excellent customer service skills; active listening and attention to detail.
  • Computer literacy in Windows-based applications.
  • Basic understanding of medical terminologies, hospital operations, and payment processing is an advantage.
  • Willing to work on-site in Bonifacio Global City, Taguig and in shifting schedules.
