Role Description

This is a full-time on-site role in Quezon City for a Medical Social Worker. The Medical Social Worker will provide patients and their families with social, emotional, and resource support. Key responsibilities include assessing patient needs, managing cases, developing discharge plans, facilitating access to social services, and collaborating with healthcare teams to ensure quality care. The Medical Social Worker will address psychosocial challenges to promote positive patient outcomes and well-being.

Qualifications

  • Experience in Medical Social Work and Social Work practices
  • Proficiency in Discharge Planning and Case Management to support patients and their families
  • Knowledge of Social Services resources and effective advocacy on behalf of patients
  • Strong communication, interpersonal, and problem-solving skills
  • Ability to work collaboratively with healthcare teams and stakeholders
  • Bachelor's or Master's degree in Social Work or a related field
  • Relevant licensure or certification as a Social Worker (as required by local regulations)
  • Experience in a healthcare setting is preferred
