PRIMARY FUNCTION
We are seeking an experienced and dedicated Patient Consulting Team Lead to oversee our Patient Consulting - Registered Nurse team in a telehealth setting. This role combines clinical expertise in patient assessment and chronic disease management with leadership responsibilities, including team supervision, performance management, and operational oversight.
The Team Leader will provide guidance and support to a team of Registered Nurses, ensuring high-quality, patient-centered care and adherence to clinical protocols. The ideal candidate has strong leadership skills, thrives in a fast-paced environment, and demonstrates a passion for mentoring and team development.
KEY RESPONSIBILITIES & DUTIES
Leadership & Team Management
- Supervise a team of Registered Nurses delivering telehealth consultations.
- Provide coaching, mentoring, and ongoing performance feedback to ensure team success.
- Monitor individual and team KPIs, driving improvements in efficiency and quality of care.
- Conduct regular one-on-one check-ins and team meetings to support collaboration and knowledge sharing.
- Handle escalated clinical or operational concerns and ensure timely resolution.
- Collaborate with management and cross-functional teams to implement best practices and process improvements.
Patient Assessment and Eligibility
- Oversee comprehensive telehealth patient assessments, including mental health evaluations.
- Guide the team in conducting structured pre-assessments and risk screenings.
- Ensure accuracy in assessing medical histories and medication regimens for treatment eligibility.
Patient-Centered Care & Clinical Oversight
- Promote non-judgmental, patient-focused care in chronic disease and medicinal cannabis management.
- Liaise with prescribers and ensure proper communication flow between patients and the clinical team.
- Support the team in responding to patient inquiries and ensuring appropriate follow-ups.
- Provide clinical advice and ensure documentation is accurate, timely, and compliant.
Administrative & Operational Support
- Coordinate general clinic administrative duties in collaboration with other departments.
- Support onboarding and training of new nursing team members.
- Contribute to continuous quality improvement initiatives and policy updates.
Qualifications, Skills, and Experience:
- Registered Nurse with current AHPRA registration (no practice restrictions).
- Minimum 5 years post-graduate clinical nursing experience, with at least 1–2 years in a leadership or supervisory role.
- Experience in telehealth or virtual care environments.
- Exposure to mental health nursing or medicinal cannabis treatment is an advantage.
- Strong background in patient assessment, care planning, and chronic disease management.
- Proficient in systems and digital tools used in healthcare settings.
- Excellent communication, problem-solving, and interpersonal skills.
- Proven ability to lead and inspire high-performing teams.
- Strong organizational and time management skills with the ability to prioritize in a dynamic environment.
