Job Summary

The Patient Care Coordinator is responsible in assisting the referral process across multiple facilities within a designated region or product segment. The role ensures compliance with policies, facilitates coordination between healthcare providers, specialists, and patients, and supports standardization of referral practices to improve efficiency and enhance patient care within the region.

Responsibilities


• Manage the referral process from start to finish, including reviewing medical records, verifying insurance requirements, and requesting missing documentation from healthcare providers.

• Intake patient claims, ensuring all required documents are present and accurate prior to processing.

• Maintain working knowledge of DME products offered.

• Coordinate directly with patients to schedule fittings and ensure clear instructions and follow-up care.

• Ensure consistency and standardization of referral procedures across locations.

• Monitor referral workflows, identifying and addressing bottlenecks or inefficiencies.

• Ensure referrals comply with regulatory requirements and organizational policies.

• Facilitate communication and collaboration between referring providers, specialists, and healthcare entities.

• Support internal patient care coordinators and/or intake staff across facilities.

• Ensure patients receive clear information and support regarding the referral process and follow-up care.

• Address patient concerns and inquiries related to referrals in a timely and professional manner.

• Maintain accurate records of referral activities and generate reports on referral trends, utilization, and outcomes.

• Collaborate with leadership to implement regional strategies for improving referral processes and patient care.

Qualifications and Requirements


• Basic understanding of healthcare delivery systems and referral processes.

• Demonstrates organizational and multitasking skills with attention to detail.

• Applies analytical and problem-solving skills to resolve challenges and implement solutions.

• Capable of effectively communicating with healthcare providers, staff, and patients.

• Clear and coherent both in written and verbal communication skills in English.

Screening Criteria


• Minimum of two (2) years of experience in healthcare administration, care coordination, or referral management.

• Must have stable employment history.
