The Home Health Clinical Intake Coordinator is responsible for reviewing and evaluating patient referrals to determine eligibility for home health services. This role involves close coordination with healthcare professionals, patients, families, and insurance providers to ensure a seamless start of care process.

Responsibilities

  • Review and evaluate patient referrals to assess eligibility for home health services
  • Schedule initial assessments and coordinate with clinicians to initiate patient care
  • Collect and verify patient demographics, insurance information, and medical records
  • Ensure all intake documentation is accurate, complete, and compliant with regulations
  • Act as a liaison between patients, families, providers, and payers to address questions or concerns
  • Resolve issues that may arise during intake, such as insurance authorizations or scheduling conflicts
  • Maintain compliance with organizational policies and applicable federal/state regulations

Requirements:

  • Active Registered Nurse (RN) license required
  • Minimum of 1 year of experience in Home Health Intake
  • Proficient in using of Electronic Medical Records (EMR)
  • Demonstrated experience handling inbound and outbound calls in a clinical or administrative setting
  • Flexible availability, including willingness to work night shifts
  • Immediate availability to begin employment
