Job Summary:

Nephrology Center of Caloocan- Edsa is seeking a skilled and dedicated Hemodialysis Head Nurse/Nursing Care Coordinator to join our team at the branch located in 522 EDSA, Brgy. Biglang Awa, Caloocan City. This role plays a vital role in overseeing and coordinating nursing care activities within the hemodialysis unit, ensuring the delivery of high-quality patient-centered care.

Key Responsibilities:

1. Leadership and Supervision:

2. Provide effective leadership to the nursing team, fostering a positive and collaborative work environment.

3. Supervise and guide nursing staff, ensuring adherence to established protocols, standards, and best practices.

4. Patient Care Coordination:

5. Coordinate and manage patient care activities, ensuring safe and effective hemodialysis treatments.

6. Assess patients' needs, develop individualized care plans, and monitor treatment progress.

7. Treatment Oversight:

8. Oversee hemodialysis procedures, ensuring proper machine setup, patient assessment, and treatment documentation.

9. Address any issues or complications that arise during treatment sessions.

10. Training and Education:

11. Provide orientation and training to new nursing staff, ensuring they are well-equipped to deliver competent care.

12. Conduct ongoing training sessions to enhance nursing skills and knowledge.

13. Quality Assurance:

14. Monitor and evaluate the quality of patient care, implementing continuous improvement initiatives as needed.

15. Ensure compliance with infection control, safety protocols, and regulatory standards.

16. Communication and Collaboration:

17. Collaborate with the medical team, healthcare providers, and support staff to ensure seamless patient care delivery.

18. Communicate effectively with patients and their families, addressing their concerns and providing education.

19. Documentation and Reporting:

20. Maintain accurate and detailed patient records, treatment logs, and reports.

21. Prepare reports on patient outcomes, quality metrics, and nursing performance.

22. Emergency Response:

23. Lead the nursing team in responding to emergencies and implementing appropriate interventions and protocols.

Qualifications:

1. Registered Nurse with a valid license from PRC.
2. Minimum of 2 years of nursing experience, with at least 1 year in hemodialysis or nephrology care.
3. Leadership experience or demonstrated ability to lead and supervise a nursing team.
4. Strong clinical skills and proficiency in hemodialysis procedures and equipment operation.
5. Excellent communication and interpersonal skills.
6. Dedication to patient-centered care, safety, and quality.
7. Proficient in medical documentation and record-keeping.
8. Willingness to work in a dynamic and fast-paced healthcare environment.

Working Conditions:

The Hemodialysis Head Nurse/Nursing Care Coordinator will primarily work at the Nephrology Center of Caloocan branch located in 522 EDSA, Brgy. Biglang Awa, Caloocan City.
