About the Role:
We are looking for a Patient Care Coordinator who will support patients and their families by facilitating care coordination, appointment scheduling, and communication between healthcare providers. This role plays a key part in ensuring a smooth patient experience and proper documentation of care plans.
- Support care plans by providing administrative assistance and facilitating appointment scheduling, referrals, and follow-ups.
- Serve as a patient and family resource while adhering to established care coordination protocols.
- Manage documentation and data, including progress notes and preparation of team reports.
- Coordinate logistics for Care Managers—handle assignments, scheduling, and written communication.
- Escalate clinical concerns appropriately to ensure quality patient outcomes.
- Assist with external coordination, including community services and managing insurance-related processes such as prior authorizations.
Must-Have:
- Experience in a healthcare-related environment, working with patients, families, and healthcare providers.
- Excellent organizational and communication skills, both verbal and written.
- Strong interpersonal skills and the ability to work collaboratively with physicians, patients, and interdisciplinary teams.
Preferred:
- Experience in case management, ambulatory care, or transition care settings.
- Familiarity with medical terminology and insurance processes.
- Background in administrative or coordination roles within hospitals, clinics, or healthcare BPOs.
- Graduates of Nursing, Medical Technology, Allied Health, or related fields are encouraged to apply.
- Candidates with experience in care coordination, utilization review, case management, or patient relations are highly preferred.
