Duties and Responsibilities:
  • Records Management
  • Collects, files, and maintains patient medical records in an organized and confidential manner.
  • Ensures completeness and accuracy of patient charts before filing or release.
  • Retrieves records promptly upon authorized request.
  • ICD-10 Coding and Documentation
  • Applies accurate ICD-10 coding for diagnoses and procedures based on physician notes.
  • Reviews and verifies coding accuracy in compliance with DOH and PhilHealth requirements.
  • Updates patient records with coded data for billing, reporting, and statistical purposes.
  • Data Reporting and Compliance
  • Prepares reports on morbidity, mortality, and other health statistics as required by DOH.
  • Ensures compliance with hospital policies, confidentiality laws, and record retention standards.
  • Coordination and Communication
  • Coordinates with doctors, nurses, and billing staff regarding record completion and coding.
  • Assists in audits and evaluations of medical records.
  • Administrative and Support Functions
  • Monitors and maintains supplies, forms, and record materials.
  • Participates in continuous training and professional development.
Qualifications:
  • Education: Bachelor's degree in medical Records Management, Health Information Management, Nursing, or related field
  • License/Certification:
  • Registered or Certified ICD-10 Coder (from DOH or accredited institution)
  • Experience: At least 1 year of experience in a hospital or medical records setting (preferred)
Skills:
  • Proficient in ICD-10 coding and medical terminology
  • Strong organizational and attention-to-detail skills
  • Knowledge of confidentiality standards and data privacy laws
