A. Records Management
Maintain and update patient medical records and hemodialysis treatment logs.
Ensure completeness, accuracy, and confidentiality of all patient information.
File and retrieve patient charts and records efficiently.
Encode and track dialysis treatment sessions and laboratory results in the hospital system.
B. ICD-10 Coding and Documentation
Assign appropriate ICD-10 codes to diagnoses and procedures based on medical documentation.
Ensure coding accuracy for PhilHealth and insurance claims processing.
Assist physicians and nurses in proper documentation for coding and billing compliance.
C. Reporting and Coordination
Prepare and submit statistical and medical record reports required by DOH and PhilHealth.
Coordinate with billing, nursing, and medical departments for patient record updates and claims.
Assist in audit and review of medical records to ensure compliance with hospital standards.
D. Confidentiality and Compliance
Maintain strict confidentiality of all patient records in accordance with Data Privacy and DOH standards.
Follow hospital policies and infection control protocols within the dialysis unit.
Qualifications:Education:
Bachelor's degree in health information management, Medical Records Administration, Nursing, or any related allied health course.
Certification:
Must possess a valid ICD-10 Coding Certificate (from DOH-accredited or recognized institution).
Experience:
Preferably with at least 6 months to 1 year of experience in medical records or dialysis center documentation.
Skills:
Knowledge of ICD-10 coding and medical terminology
Proficient in MS Office and hospital information systems
Organized, detail-oriented, and able to maintain data confidentiality
Good communication and coordination skills
