As a critical member of our healthcare team, the Outpatient Program Coordinator plays a pivotal role in ensuring seamless care coordination for clients.

Job Responsibilities:

  1. The primary function is to serve as the main point of contact for assigned outpatient clients and their families, providing support with non-clinical needs and promoting positive treatment outcomes.

  2. Coordinate all in-person and virtual therapy sessions, psychiatric appointments, and calendar management across clinical schedules.

  3. Accurately track and report no-shows, cancellations, and reschedules.

  4. Regularly monitor client participation in scheduled services and follow up on attendance, engagement, and any missed appointments.

  5. Efficiently coordinate client intakes and discharges, ensuring timely completion of all necessary documentation.

  6. Identify potential barriers and unmet needs in areas such as housing, education, transportation, healthcare, legal services, and other social determinants of health, linking clients to relevant resources.

  7. Maintain ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.

  8. Participate actively in interdisciplinary team meetings to review and support treatment plan goals.

Qualified candidates will possess at least 2+ years of experience in behavioral health, social services, or case management, along with strong written and verbal communication skills and proficiency in Google Workspace and/or Microsoft Office.
