
Client Support Specialist

The primary objective of an Outpatient Case Manager is to deliver exceptional, client-focused support throughout the treatment process. This encompasses intake, active care coordination, appointment scheduling, and discharge planning.
Key responsibilities include:
- Serving as the main point of contact for assigned outpatient clients and their families
- Coordinating appointments and ensuring accurate calendar management
- Tracking and reporting no-shows, cancellations, and reschedules
- Maintaining ongoing communication and documentation regarding care coordination efforts
- Participating in interdisciplinary team meetings to review and support treatment plan goals
To be successful in this role, candidates must possess:
- At least 2 years of experience in behavioral health, social services, or case management
- Strong written and verbal communication skills
- Proficiency in Google Workspace and/or Microsoft Office
Preferred qualifications include an Associate's or Bachelor's degree in Social Work, Psychology, or a related field, experience working with adolescents and families, and knowledge of Medicaid, state behavioral health systems, and community-based services.
