**Job Description**

The patient engagement specialist role plays a critical part in bridging the gap between healthcare providers and patients. This position requires exceptional communication skills, empathy, and the ability to build rapport with individuals from diverse backgrounds.

The ideal candidate will possess strong analytical skills, be detail-oriented, and have a proven track record of successfully navigating complex systems.

Key Responsibilities:
  • Pre-Enrollment Communication & Conversion:
    • Initiate contact with prospective patients via phone, SMS, or email to guide them through the pre-enrollment process.
    • Ensure accurate and timely completion of pre-enrollment documentation.
  • Patient Engagement & Relationship-Building:
    • Act as a warm, informed first point of contact for patients, making them feel confident and supported throughout their journey.
    • Provide clear and empathetic guidance on the process, addressing questions and concerns in a professional manner.
  • Operational Coordination:
    • Update and maintain accurate patient data in CRM and health system tools.
    • Monitor pre-enrollment funnel metrics, identify bottlenecks, and escalate issues proactively to optimize workflows.
