The Care/Provider Navigator plays a pivotal role in expanding our presence in the healthcare industry by guiding and supporting providers to ensure seamless onboarding, engagement, and collaboration for improved patient outcomes.

Key Responsibilities
  • Care Coordination (80%)
    • Establish strong relationships with patients through personalized interactions ensuring a welcoming experience.
    • Collect, verify, and update patient information accurately while maintaining confidentiality.
    • Schedule appointments efficiently and conduct initial screenings to direct patients appropriately.
    • Manage patient records in electronic medical record systems ensuring compliance with HIPAA regulations.
    • Respond to patient inquiries via phone, email, and chat with empathy and clarity providing solutions whenever possible.
    • Provide pre-appointment instructions and ensure patients understand documentation and insurance requirements clearly.
    • Coordinate with healthcare providers, insurance teams, and staff to ensure smooth patient flow.
  • Provider Partnership (20%)
    • Conduct proactive provider outreach and present our value proposition to acquire strategic partnerships.
    • Develop and maintain accurate records of outreach activities in customer relationship management software.
    • Track and report key performance indicators such as calls, emails, meetings, and conversion rates.
