Job Overview

The role of a Care Navigator is crucial in expanding the company's presence in the healthcare industry. This position focuses on guiding and supporting providers, ensuring seamless onboarding, engagement, and collaboration for improved patient outcomes.

Key Responsibilities
  • Care Coordination (80%)
    • Serve as the first point of contact, ensuring a professional and welcoming patient experience.
    • Collect, verify, and update patient information accurately while ensuring confidentiality.
    • Schedule appointments efficiently and conduct initial screenings to direct patients appropriately.
    • Manage patient records in electronic medical record systems, ensuring compliance with regulatory requirements.
    • Respond to patient inquiries via phone, email, or chat with empathy and clarity.
    • Provide pre-appointment instructions and ensure patients understand documentation and insurance requirements.
    • Coordinate with providers, insurance teams, and staff to ensure smooth patient flow.
  • Provider Partnership (20%)
    • Conduct provider outreach and present the value proposition to acquire provider partnerships.
    • Handle high outbound call volumes effectively.
    • Maintain accurate outreach records in CRM systems.
    • Track and report key performance indicators (KPIs) such as calls, emails, meetings, and conversion rates.
    • Share insights on prospect feedback and market trends to improve strategies.
Week 1 – Immediate Tasks & Onboarding
  • Complete product and industry training.
  • Gain proficiency in CRM and outreach tools.
  • Develop a target account list and begin initial outreach efforts.
  • Shadow sales calls to understand prospect pain points.
  • Conduct mock prospecting sessions for script refinement.
Must-Haves
  • Proven experience in care coordination, particularly in physical healthcare settings.
  • At least one year of outbound sales experience, preferably in healthcare.
  • Strong cold-calling and email outreach skills.
  • Proficiency with CRM software, LinkedIn Sales Navigator, and other relevant tools.
  • Excellent verbal and written communication skills.
  • Ability to thrive in fast-paced, metrics-driven environments.
