Healthcare Liaison Opportunity

We are seeking a skilled healthcare professional to serve as the primary liaison between patients and caregivers.

This role requires proactive communication, organizational skills, and strict confidentiality.

The Opportunity:

The Healthcare Liaison serves as the bridge between patients and care teams, ensuring high-quality care and patient satisfaction.

This position is remote and offers flexibility, allowing you to work from anywhere with a reliable internet connection.

Key Responsibilities:

Patient Care Coordination:

  • Serve as the primary liaison between aides and patients.
  • Monitor and maintain care quality by completing measures and quality assurance activities.
  • Keep informed of patient care issues, including complaints and changes in patient status.
  • Ensure schedules reflect vendor authorization and patient preferences.

Communication & Documentation:

  • Communicate with patients, families, physicians, and care teams regarding scheduling and plan-of-care updates.
  • Maintain accurate documentation for all caregivers and patients.
  • Handle incoming calls, emails, and faxes from physicians and clients regarding patient status or inquiries.
  • Log and maintain proper documentation of events and patient progress.

Collaboration & Support:

  • Work closely with the patient care coordinator on scheduling and operational issues.
  • Manage phone calls and correspondence in a supportive manner.
  • Provide additional support and responsibilities as assigned.
Qualifications:
  • Registered Nurse with NY License.
  • Bilingual in Spanish.
  • Minimum 3 years of case management or related experience.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and customer service skills.
  • Ability to handle difficult situations professionally.
