
Patient Care Coordinator Specialist

This role is ideal for someone who excels in delivering exceptional patient care and has strong communication skills.
Key Responsibilities:- Answer incoming calls promptly and professionally, providing accurate information to patients.
- Make outbound calls to patients for appointment reminders, follow-ups, and rescheduling, ensuring seamless communication.
- Respond to patient text messages and emails with timely and accurate information, maintaining a high level of service.
- Schedule and reschedule patient appointments according to provider availability, manage the triage process, and assist in creating new patient referrals.
- Handle faxes, upload documents, reroute faxes to appropriate departments, and maintain patient records by mirroring data between systems.
- Multi-task across phone, email, and text communication while maintaining a high level of service, following up on pending authorizations, lab results, or documentation as directed.
- Proven experience in a healthcare setting, preferably in patient coordination or a similar role.
- Excellent English communication skills, both written and verbal, with a strong ability to communicate effectively with patients and healthcare providers.
- Strong organizational and multi-tasking abilities in a fast-paced environment, with a high attention to detail and accuracy when handling sensitive patient data.
- Familiarity with EHR/EMR systems, such as Practice Fusion and AdvancedMD, or willingness to learn and adapt.
- Proficiency with office software, email systems, and document management, with a focus on delivering exceptional patient care.
